CF were commissioned by the Scottish government to develop a case for national investment in integrated care. Our work focused in several areas. First, developing a case for change for the move to integrated care. Second, developing the evidence based clinical model and the enabling requirements, in particular workforce, data and estates. Third, modelling the potential impact and investment required and lastly, developing a path to implementation across Scotland. In addition to working with the centre, we worked with three local areas in order to refine our work, and for those to be the ‘pathfinders’ for implementation.
